For a relaxing trip it is always a good idea not to rush, but if you want to see a bit more you may consider allocate less time for Yokohama and Hakone. On a tight schedule some just do a day each for the 2 places, but 2 days each for Hakone and Yokohama should be quite enjoyable for a more in-depth look at either place.

2 days in the Hakone area can include a visit to the Gotemba Premium Outlets. As for Mt. Fuji, it depends what you want to do. Assuming you just want to see it, with luck you can see it in Hakone without setting aside extra days to go for it. Seeing Mt Fuji is a hit-and-miss affair and unfortunately most of the time it is a miss as it is mostly hidden behind clouds. If you want a closer encounter, spare a day or two and go to the Fuji Five Lakes area.

If you are into manga and anime stuff, another worthy place in Yokohama is the Anpanman Children's Museum

Note that the Fujiko F. Fujio Museum (Doraemon Museum) is not in Yokohama but in Kawasaki and can be done as an easy side trip from Tokyo.

Also note that the 1:1 Gundam is being installed again in Odaiba and will be officially opened on Apr 19 this year.

Apart from the 2 days you have allocated for Disney, 5 days for the various districts in Tokyo should be quite relaxing and not too rush. Just remember to add Odaiba to the list.

If you can spare a day or two and don't mind going out of town, consider doing day trips to Nikko and Kamakura.
