Is it a little too long to spend 5 days in Tokyo only?. .Wanna go to Hokaido too but time too short right?

No, I don't think so, especially if you use Tokyo as a base for a day trip or two.

Usual day trip recommendations are Nikko, Kamakura, Hakone and/or Kawaguchiko.

I also think it depends a little on what you mean by "5 days". Is that including the day you arrive and the day you leave? At what times of the day?

What do you want to see in Hokkaido? You could theoretically fly to Sapporo and back and spend a day and a half there or so, but it is probably too short for most people. I'd usually suggest to just concentrate on Tokyo and the surrounds unless you have more time.

You can also consider a side trip from Tokyo to Kyoto, even if you just go for one night (and two whole days) but for some people this would also be too much for five days.
